eCare Companion

eCareCompanion is a patient engagement tablet-based app. It is an intuitive and personalized companion for chronically ill patients to track their health and stay connected with their care teams. With a tap of the screen, chronically ill patients launch eCareCompanion and easily begin providing vital information to caregivers. Patients answer survey questions and enter requested measurements such as weight, blood pressure, temperature and oxygen saturation. They are reminded of pre-assigned health tasks and may be asked for additional information the clinician needs to make an assessment. It connects with a cloud-based telehealth management platform (eCareCoordinator), so clinicians can remotely monitor patients and make timely care decisions.
